INSURANCE AND FEES

There are two methods of payment for treatment:
- Cash
- Insurance
The fees that apply to you will depend on:
- Type of treatment
- Payment by insurance or cash (if you pay on the same day of treatment, a discount is given)
Some general comments about
how insurance works:
- If you choose to use insurance payment, Roots WellCare, PA will
submit claims to your insurance on your behalf. Our billing company
is Physicians Resource Network (PRN) in Golden Valley, MN. Please contact
PRN at 763-545-2842 with your questions.
- You are responsible for understanding what your insurance plan does
and does not cover. We will verify coverage with your insurance before
initiating treatment. To do this, we need information from your insurance
card.
- You are ultimately responsible for payment of your bill if your
insurance company denies payment to Roots WellCare, PA.
- Insurance companies offer many different plans for subscribers.
Two people both may have Blue Cross Blue Shield, but their plans' coverage may be drastically different.
- The rates billed by Roots WellCare, PA for insurance are higher
than for cash payment due to costs associated with submitting the
claim to your insurance.
Treatment types that are often covered by insurance:
- Chiropractic new client consultation and examination
- Manipulation of the spine and other joints
- Additional therapies: ultrasound, muscle therapy, exercise therapy
and instruction, ice or heat application, manual traction therapy
Treatment types that are sometimes covered by insurance:
- Acupuncture
- Foot orthotics
Treatment types and products that are rarely or never covered
by insurance:
- Homeopathic consultation and remedy
- Laboratory testing
- Nutritional supplements
- Ice packs
- Orthopedic supplies: back rests, back supports, etc.
FREQUENTLY ASKED QUESTIONS (FAQS)
1. Is Dr. Breunig a Blue Cross Blue Shield provider?
Yes. Dr. Breunig is in the Aware Gold network. She is not in the Blue
Select network.
2. Is Dr. Breunig a ChiroCare provider?
No. For your information, Medica and Health Partners distinguish between “in-network” providers and “out-of-network”
providers. “In Network” refers to the ChiroCare network.
When you contact Medica or Health Partners, please ask whether you have
“out-of-network” benefits.
3. Is Dr. Breunig a Medicare provider?
Yes. Medicare will pay for chiropractic manipulation to the
spine only. Medicare will not pay for other services when prescribed
or performed by a chiropractor. These services include lab tests, x-rays,
treatment therapies besides manipulation, nutritional supplements, and
physical examinations.
4. What questions should I ask my insurance company when I
call to ask about benefits?
Please ask the following questions:
- What is my effective date of coverage?
- Is my plan on a calendar year, or other (for example, September
1 to August 31)?
- What is my deductible: per individual, and per family?
- Has my deductible been met yet for the year?
- Do I have a co-payment? If so, what is it?
- After my deductible has been yet, what percentage does my insurance
pay?
- Do I need a referral from my medical doctor to see a chiropractor?
